Officials confirmed that the toxicology report indicated a mix of heroin and alcohol to be the cause of Cory Monteith's death. The autopsy and toxicology analysis, concluded that his death was accidental as the actor was not trying to take away his own life.
Back in May the actor had checked into a facility to treat his drug addiction according to reports. He said in 2011 that he had gone to rehab once before when he was 18. He was 31 years old at the time of his death.
