An Eaglemed helicopter crashed near Talihina Oklahoma shortly after taking off Tuesday night. Three crew members were injured and the patient being emergency med-evacuated was killed. This is the third crash in three years for the Eaglemed Company, based in Wichita Kansas.

The Commission on Accreditation of Medical Transport has placed the accreditation of Eaglemed on hold following Tuesday night's crash. Crew members onboard the Eaglemed chopper were pulled from the wreckage but did not require medical treatment.

The executive director of the Commission on Accreditation of Medical Transport, Eileen Frazer says they are looking into the Eaglemed crash and trying to determine if there is a pattern of unsafe conditions that links the three crashes since 2010.

"Currently Eaglemed is working with the National Transport Safety Board and FAA, we're helping them with their investigation by providing the information they need from us," said Eaglemed spokesman Robbie Copeland to the Associated Press.

This is the second Eaglemed crash in Oklahoma this year. The first crash happened in February, a nurse and the pilot were both killed and a medic was critically injured.

"It takes a while before we determine the cause of an accident," NTSB spokesman Keith Holloway said to AP. "We don't look at the company, we look at issues. If there is something that reveals itself, we'll look at it."

The FFA is still investigating the February crash as well as Tuesday night's crash.
